Concept Synthesis Paper on Personal Nursing Philosophy
The paper deals with humanistic nursing philosophy that guides the family nurse practitioner (NP) in the daily nursing efforts and the decision-making. The 10-year practice experience and permanent education of the registered nurse seems to be the foundation of the professional excellence. The personal experience has determined the humanistic nursing philosophy with the focus on the individual and social needs of a patient. The nurse multitasking, caring, leadership and communication excellence appear to ensure the integrality of the patient, environment, nurse and nursing. The implementation of specific to the practice family-based concepts is considered the necessary practice for the patients safety and satisfaction. The relationships between the identified concepts resulted in seven propositions of the nursing framework.
Keywords: patient-centered nursing, holistic care, family concepts, integrality
Advanced nurse is the most trusted and needed profession to solve health care crisis. Current conditions in the health care industry require permanent improvements in the quality of services within nursing. The combination of humanistic nursing philosophy and the holistic evidence-based practice predispose the establishment of the nurse excellence. The latter incorporates mastering of basic and innovative treatment techniques, life-long high-quality education, and leadership activities. These are the features enabling a nurse practitioner to identify positive feedbacks between four concepts: patient, environment, nurse and nursing. The identification and implementation of the family concept, family development, the concepts of stress and fear distinguish the propositions of family nursing.
As a registered nurse who practiced for more than ten years in the health care industry, I strongly commit to the caring concept and social mission of nursing. The latter involves humanistic approach in professional medical, advocating, educating and leadership activities contributing to the holistic and patient-centered care. Professional experience in the variety of nursing roles established a comprehensive vision on modern nursing and nurse power. I started my professional career in Friendly Hands Inc. in 2003-2006, where I obtained the primary skills of compassionate daily care of the clients basic needs and learned to communicate positively with clients. The experience in the referral management contributed to forming a comprehensive vision of the health care system and seeing the opportunities for the patient in it. Handling patients needs and guiding them in the health care unit meant working with booking, insurance, NHS and Book information. Personal excellence in basic medical services concerns the positions of phlebotomist, surgical coordinator, and registered nurse. By fulfilling the role of a Nurse Case Manager/Nurse Reviewer, I advanced in the advocating mission, while promoting and maintaining the optimal treatment and healthy lifestyles among patients. The current position of a Nurse Case Manager for a Medical Insurance company makes me a member of the management team in the healthcare system and enhances my understanding of the legislative and financial aspects of the affordable care.
Bachelors degree in the Florida University-Miami and professional training programs in radiology and phlebotomy services contribute to the professional development in advanced nursing and evidence-based practice. Clinical rotation hours, besides basics of nursing, included pediatric, adult, maternal/infant and psychiatric nursing. Professional nursing leadership was another essential course for becoming modern and powerful health care professional. During this time, I managed to administer essential medical procedures, assess patient needs, monitor treatment plans, and provide family nursing services. On the way, delivery of patient-centered care became possible while following the nursing theory and research activities. The appropriate implementation of theoretical knowledge requires identification and understanding of the patient, nurse, health, and environmental paradigms in everyday professional activity and decision-making.
The health-illness continuum can be resolved in a humanistic way while being the primary object of advance nursing. The idea focuses on the establishment of effective feedbacks and integrality between patient, nurse, health, and environment. The ideas of caring and cognitive nursing theories provide a comprehensive insight on the interactions of those components in dynamics.
The provision of health care continuity refers to incorporation of an individual and his/her needs into the health care system as an indicator of its efficiency. The persons consciousness and his/her health behavior as a patient form professional nursing efforts and outcomes. The theories by Rogers and Newmann identify a patient as an individual being who is an integral part of his/her environment and human-environment interactions (Masters, 2014). Accordingly, the patients health-illness continuum reveals with the development and the quality of the mentioned interactions (Bateman & Merryfeather, 2014). Besides, the nursing recipient acts in response to the established configuration of mind-body-spirit. The awareness of the components of the nurse case management helps to establish the patients goals. Such combination initially depends on personal attitudes and genetic predisposition. Still, mostly the environment shapes patients health behavior and the parameters of the health care feedbacks.
The environment includes the reflection of the patients social and personal appearance as well as the health care decisions. The individuals improvement in the health status corresponds to the attitude towards the environment through the expansion of self-consciousness. Hence, the environment that incorporates the conditions in which human lives predisposes the initial changes in the health status as well (Masters, 2014, p. 56). For instance, the acknowledgment of the Asian ethnic background of the patient enabled to understand the patient response to medical procedures. The nurse and nursing are the concepts identify the changes needed in the persons environment to optimize the health care systems and the patients independence.
Accordingly, the modern role of the nurse includes the necessity of becoming a change agent. Initially, a nurse is also an individual with personal preferences, strength, and weaknesses that guide the professional activity. Life-long professional and personal development contribute to the nurse autonomy in helping people. The nurse power leads to the patients awareness and independence in the health management (Masters, 2014, p. 51). This knowledge strengthened my motivation for the professional leadership and training activities. On the way, the nurse continuously interacts with the environment. Consequently, the nursing work environment can be as important as the range of skills and personal philosophy of nursing (Jarr?n, 2012). All the mentioned strengths of an advanced nurse realize through patient-centered nursing that fills the gaps in the health care delivery.
The position of nursing in the field expands with every year reflecting the need for comprehensive health care. Accordingly, a range of professional skills, such as diagnostics, monitoring, documentation management, treatment, and promotion, is just the basis for efficient services. All the areas of expertise give meaning for the patient while the nurse delivers them in a humanistic way. Still, to eliminate the subjectivism of humanistic approach the NP has to maneuver between art and science of nursing. Hence, nursing excellence should consist of education, science, support, leadership and collaboration with the members of health care system (Duff, 2011). All the elements are permanently changing, requiring increased awareness of the NP. Besides, the integrality of the nurse-patient-environment is the issue contributing to consciousness and integrity of nursing. Deep insight into the concept of advanced nursing contributed to the self-management, while handling emergencies. The mentioned above paradigms become applicable on several levels of health care system in family nursing.
Family nursing requires implementation of concepts specific to the practice, including family unit, family nursing, family homeostasis, and supportive ones. The concepts help to determine the mission of the family NP and his/her skills while delivering the family-centered care. The latter serves the lowest unit with affordable and quality nursing services.
The concept of the family unit is the fundamental issue for the family nurse practitioner that aims to explain the role of family community in support, provision, and promotion of health care. The family, as the smallest social environment, indicates the possibilities for individual wellness. The issue transmits to the health care processes since the strengths and vulnerabilities of the family form the caregiving (Denham, Eggenberger, Young, & Krumwiede, 2016, p. 165). Hence, the family nurse practitioner tends to be the health care advocate in the process of family nursing.

The concept of family nursing builds on several stages of nurse-patient-environment interaction. On the first level, the nurse has to understand individual interests of the patient and his/her family members. Empathetic approach seems to be necessary to distinguish the environmental visions of the health care receivers from the objective status of health-illness continuum. On the next level, family system becomes susceptible to health care providers. Collaboration and access to the efficient treatment and insurance resources defines the quality of feedbacks. The family-centered nursing serves as the bridge that enables health care intervention to reach the needs and possibilities of the family members and caregivers (Denham et al., 2016, p. 172).
Mentioned activities should result in the family homeostasis, since it is a necessary condition of the health care changes. The concept of family homeostasis manifests in the identification and professional support of the family, enabling caregiving. The nursing depends on familys interdependence, stability and feedbacks (Kim & Rose, 2014). Accordingly, the equilibrium in the family health care unit constitutes the optimal environment and the goal of the family nursing. The concept includes strengths, stress, and fear of the family that influence its development in the health care context.

Practice experience in a variety of nursing roles prepares the transformation of a registered nurse into the family nurse practitioner. Advanced professional development involves the generation of the philosophy framework that relies on humanistic and holistic attitude to caregiving. Holistic philosophy involves the integrality of the four dynamic metaparadigms that are built on nurses leadership and mentorship. The efficiency of the holistic nursing depends on the maintenance of changes needed for all nursing paradigms. Family concepts direct nursing philosophy to the needs of family communities as the elementary healing environments.
